These Terms and Conditions (“Terms”) govern your access to and use of the website, services, content, tools and features made available by Live Longer Better Pte. Ltd., operating as DementiaCare Connect (“DementiaCare Connect”, “we”, “us” or “our”).

By accessing or using our website or any services made available by us, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, you should not use our website or services.

1. About DementiaCare Connect

DementiaCare Connect is a Singapore-based platform that may make available services and support tools relating to dementia caregiving, including:

  • educational content, guides and resources;
  • tele-consultation or support sessions with independent allied health, advisory or support professionals;
  • enquiry, contact, booking, waitlist or demo request forms; and
  • JUMI or other digital or AI-supported tools, where made available by us.

Unless expressly stated otherwise, DementiaCare Connect provides the platform, coordination layer, content and related support tools, but does not provide emergency medical services or medical diagnosis.

2. Important Disclaimers

2.1 Not for emergencies

DementiaCare Connect is not intended for emergencies. If you believe that you or another person is in immediate danger, or if urgent medical attention is required, call the emergency services number in your location or seek immediate in-person medical assistance.

2.2 No medical diagnosis or emergency care

Our website, resources, tele-consultation services and AI-supported tools are intended to provide education, support, care navigation and non-emergency guidance only. They are not a substitute for in-person medical diagnosis, emergency care, or treatment by a licensed medical doctor or other appropriate healthcare professional.

2.3 Information you provide matters

Any guidance, support or recommendations provided through our services depend on the completeness and accuracy of the information you choose to provide. Incomplete, inaccurate or outdated information may affect the relevance or usefulness of any response, support output or guidance.

3. Eligibility and Use by Others

You represent and warrant that:

  • you are at least 18 years old; or
  • if you are under 18, you are using the website or services with the involvement and consent of a parent or legal guardian.

If you use our services on behalf of another person, including a loved one, family member or care recipient, you represent and warrant that you have the authority to do so and, where required, have obtained any necessary consent or authorisation.

4. Your Use of Our Website and Services

You may use our website and services only for lawful purposes and in accordance with these Terms.

You must not, and must not permit or assist any other person to:

  • use our website or services in any unlawful, fraudulent, abusive, defamatory, harassing or harmful manner;

5. Tele-Consultation and Professional Services

Where we make tele-consultation or support sessions available, you acknowledge and agree that:

  • such sessions may be delivered by independent allied health, advisory, counselling, coaching or support professionals engaged through or introduced via DementiaCare Connect;
  • appointment requests may be subject to the relevant professional’s availability and acceptance;
  • unless expressly stated otherwise, the professional providing the session is responsible for the professional services, advice or support they provide to you;
  • DementiaCare Connect may facilitate booking, scheduling, communications and related support, but does not guarantee the availability, suitability or outcome of any session; and
  • tele-consultation or support sessions may not be appropriate in every situation, and an in-person consultation or other support may be more appropriate depending on the circumstances.

If a professional recommends that you seek in-person medical attention, emergency assistance or another form of support, you should do so promptly.

6. JUMI and AI-Supported Tools

Where JUMI or any other AI-supported tool is made available by us, you acknowledge and agree that:

  • responses may be generated using automated systems based on the information you provide;
  • such tools are intended for non-emergency support, education, care navigation and practical guidance only;
  • such tools do not replace clinical judgement, emergency services, in-person assessment, medical diagnosis or treatment;
  • outputs may not always be complete, accurate, appropriate or suitable for your particular circumstances; and
  • you remain responsible for exercising your own judgement and for seeking appropriate professional or emergency help where needed.

We may update, suspend, restrict or discontinue JUMI or any AI-supported tool at any time.

7. Content and Resources

Content made available on our website, including articles, guides, checklists, educational materials, recommendations, links and resource listings, is provided for general informational purposes only.

We do not guarantee that any content is accurate, complete, current, suitable for your circumstances or free from error. External resources or third-party services mentioned on our website are provided for convenience only and do not constitute endorsement unless expressly stated.

8. Bookings, Fees and Cancellations

Where any tele-consultation, support session or paid service is offered through DementiaCare Connect:

  • applicable fees, if any, will be communicated at or before booking;
  • a booking is only confirmed when accepted or confirmed through the relevant booking flow or communication channel;
  • cancellation, rescheduling, refund or no-show rules may apply where communicated at the time of booking or otherwise notified to you; and
  • we may change fees or service details from time to time before a booking is confirmed.

Unless expressly stated otherwise, fees paid for completed sessions or services are non-refundable.

9. Privacy and Personal Data

Your use of our website and services is also governed by our Privacy Policy.

Where you provide us with personal data, including personal data relating to another individual, you agree that such data may be collected, used, disclosed and otherwise processed in accordance with our Privacy Policy.

10. Intellectual Property

All intellectual property rights in and to our website, branding, text, graphics, design, software, content, functionality, databases, compilations and other materials made available by us are owned by or licensed to DementiaCare Connect unless otherwise stated.

Nothing in these Terms transfers any intellectual property rights to you. You are granted only a limited, revocable, non-exclusive, non-transferable right to access and use our website and services for their intended purpose and in accordance with these Terms.

11. Third-Party Services and Links

Our website or services may include links to third-party websites, booking tools, communications tools, resource providers or professionals. We do not control and are not responsible for the availability, content, privacy practices, security or terms of any third-party service.

Your dealings with any third party are solely between you and that third party, except to the extent we expressly state otherwise.

12. Disclaimers

To the maximum extent permitted by law:

  • our website, content, services and tools are provided on an “as is” and “as available” basis;
  • we do not warrant that our website or services will be uninterrupted, error-free, secure, or free from viruses or other harmful components;
  • we do not warrant that any content, recommendation, response, guidance, professional session or support output will meet your expectations or achieve any particular outcome; and
  • we do not warrant that our website or services are appropriate for every user or every caregiving situation.

Nothing in these Terms excludes any right that cannot lawfully be excluded under applicable law.

13. Limitation of Liability

To the maximum extent permitted by law, DementiaCare Connect and its officers, employees, contractors, licensors and service providers shall not be liable for any indirect, incidental, special, consequential or punitive loss or damage, or for any loss of profits, revenue, goodwill, business opportunity, data or anticipated savings, arising out of or in connection with your use of, or inability to use, our website or services.

To the maximum extent permitted by law, our total aggregate liability to you for any claim arising out of or in connection with these Terms or your use of our website or services shall not exceed the amount, if any, paid by you directly to us for the specific paid service giving rise to the claim in the twelve (12) months preceding the event giving rise to the claim.

Nothing in these Terms limits or excludes liability for fraud, wilful misconduct, death or personal injury caused by negligence, or any other liability that cannot lawfully be limited or excluded.

14. Indemnity

You agree to indemnify and hold harmless DementiaCare Connect, its officers, employees, contractors and agents from and against claims, liabilities, losses, damages and expenses, including reasonable legal costs, arising out of or in connection with:

  • your breach of these Terms;
  • your misuse of our website or services;
  • your violation of any law or the rights of any third party; or
  • personal data, content or information submitted by you in breach of these Terms or without proper authority.

15. Suspension and Termination

We may suspend, restrict or terminate your access to all or part of our website or services at any time if:

  • you breach these Terms;
  • we reasonably believe your use may create legal, security, operational or reputational risk;
  • we are required to do so by law, regulation or a competent authority; or
  • we discontinue the relevant service.

You may stop using our website or services at any time.

Any provisions of these Terms which by their nature should survive suspension or termination shall continue in effect, including provisions relating to intellectual property, disclaimers, limitation of liability, indemnity, dispute resolution and governing law.

We may modify, suspend, withdraw or discontinue any part of our website, content, services, tools or features at any time, with or without notice.

We are not liable for any unavailability, interruption, modification or discontinuation of any part of our website or services.

Your continued use of our website or services after updated Terms are posted constitutes your acceptance of the updated Terms to the extent permitted by law.

18. Governing Law and Dispute Resolution

These Terms are governed by and construed in accordance with the laws of Singapore.

If any dispute arises out of or in connection with these Terms, the parties shall first attempt in good faith to resolve the dispute through amicable discussions.

If the dispute is not resolved within thirty (30) days after one party notifies the other party of the dispute in writing, the dispute shall be submitted to the courts of Singapore, and the courts of Singapore shall have exclusive jurisdiction.

19. General

If any provision of these Terms is held to be invalid, illegal or unenforceable, the remaining provisions shall remain in full force and effect.

Our failure to enforce any provision of these Terms does not constitute a waiver of that provision or any other provision.

These Terms, together with our Privacy Policy and any additional terms expressly incorporated by reference, constitute the entire agreement between you and DementiaCare Connect in relation to your use of our website and services.
